Patient Transfer Center

At Cabell Huntington Hospital, our commitment is to be a superior service provider to both patients and healthcare partners. We understand the importance of accepting patients from other hospitals and sincerely appreciate those who refer patients to our care. A Patient Transfer Center has been established to significantly elevate our level of service when processing your patient transfer requests. With just one convenient phone number to call for adult and pediatric patients and 24/7 servicing capability from a highly qualified staff, our goal is to seamlessly streamline the transfer process from patient acceptance to patient discharge.
The Cabell Huntington Hospital Patient Transfer Center is easy to use. Simply call 1-877-531-2CHH (2244) for around-the-clock assistance from our dedicated team of experienced critical care nurses who serve as patient transfer coordinators. When calling the Patient Transfer Center, please be prepared to provide the following information:
- Your name
- Referring physician's name
- Referring hospital's name
- Phone number
- Patient's name
- Service needed by patient
- Patient’s basic condition
The patient transfer coordinator will then:
- Arrange for patient acceptance. In the event that bed space is unavailable at CHH, the patient transfer coordinator will arrange acceptance of the patient at an alternate facility
- Arrange the patient's transportation to Cabell Huntington Hospital once the patient has been accepted
- Facilitate all paperwork requirements associated with the patient’s admission and coordinate bed arrangements
- Assume responsibility throughout the patient’s stay at CHH to periodically phone the referring hospital/physician with a patient condition report
- Phone the referring hospital/physician when the patient is discharged and mail a complete discharge summary
As an added service, referring physicians are invited to experience the convenience of tracking patients through Cabell Huntington Hospital's CARE Connect program. CARE Connect is a free service that offers non-staff physicians the ability to remotely access CHH's electronic health records system and secure their patients' lab reports, radiology reports, consult reports and discharge summaries.
