Teresa Sexton, RN, MSNeD

An employee of Cabell Huntington Hospital for 12 years, Teresa has extensive nursing experience in the areas of ICU, Cardiac Telemetry, the Emergency Room, Medical Surgical, Neurology, Renal and Pulmonary. She has served as a preceptor to students, graduate nurses and new staff members of CHH. She is an adjunct professor of nursing at Mountain State University. Teresa earned her bachelor's degree from Mountain State University and her MSNeD from Breyer State University. In her new role, she:

  • Consults on all inpatients age 65 and older
  • Assesses the patient and the nursing care to ensure that individualized, evidence-based geriatric care is being given
  • Conducts comprehensive assessments and interventions on older adults with focus on high-risk issues for the geriatric patient including cognition/mental status, functional ability, mobility, hydration, nutrition, skin integrity, pain management, bowel/bladder function, polypharmacy, substance abuse and elder abuse/neglect
  • Interacts with nursing to provide education, support, interventions and resources for optimal patient care
  • Serves as a liaison and advocate for patient and family, assisting with communication across the spectrum of care and caregivers
  • Consults with the medical team and interdisciplinary team as needed to discuss goals and plan of care
  • Promotes collaboration between medical teams, interdisciplinary services and nursing
  • Serves as a liaison with long-term care facilities who have a hospitalized resident at CHH
